In order to expedite the referral process, Glade Run Lutheran Services requests that the following information be included in the initial referral for residential placement:

• A current psychiatric evaluation recommending residential treatment as medically necessary
• Additional psychiatric or psychological evaluations if available. This will assist the admissions staff in developing a clinical and behavioral outline for presentation to the Glade Run Referral Committee
• Treatment History
• Current grade and School District, SED label if applicable.
