Blended Case Manager

Status: Full time

Requirements:

  • Bachelor’s Degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history criminal justice, theology, nursing, counseling or education required; or be a registered nurse; or a high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and 2 years experience in public or private human services with 1 year in direct client contact; or a high school diploma and 5 years of mental health direct care experience in public or private human services with employment as an ICM/BCM staff person prior to April 1, 1989.
  • One to three years working with children and/or adults in mental health setting and one to three years case management experience preferred.
  • Must have audio/visual/verbal acuity to perform essential job functions.
  • Must have physical dexterity to work in consumers homes and in the community doing activities including walking, lifting, cleaning, etc.
  • Must complete and pass agency Crisis Intervention Training for community based services.
  • Must possess excellent verbal and written communication skills.
  • Valid driver’s license and access to a reliable automobile is required.
  • Must be able to adapt hours to needs of position.  May need to work evenings and/or weekend hours periodically including on-call emergency services on a rotating basis.
  • Must have access to a telephone.
  • Must be able to remain organized despite pressured situations.
  • Must be eligible for Act 33/34 and FBI clearances.
  • Must be able to identify with the agency Mission Statement and support the agency’s Core Values.
