Family Reunification Services

father with childrenThe Family Reunification program prepares families who are anticipating the return of their child from an out-of-home placement,  to enable the successful return of that child, and prevent additional out-of-home placements.   Services to identified families traditionally would begin up to thirty (30) days prior to the date of the anticipated return of the child or children to the home, and  are continued for up to ninety (90) days upon return.  Services are provided to families in their home and community.

Case management staff provide services based upon the issues identified in the referral form and the Family Service Plan, which is developed from input from the family, the referring agency and Glade Run Lutheran Services.  Goals and objectives of the Family Service Plan would include:

  • Assistance in maintaining or re-establishing a household;
  • Finding affordable housing;
  • Budgeting;
  • Teaching parenting skills;
  • Behavior management planning;
  • Linkage and referral to community service agencies, health care providers and community support networks.
