Referral Information

Referrals to the RTF can be made by contacting the Admission department at (724) 452-4453 and asking for one of the following individuals:

• Michele Cannon x 1289

or e-mail admissions@gladerun.org

In order to expedite the referral process, Glade Run Lutheran Services requests that the following information be included in the initial referral for RTF placement:

• Current Psychiatric Evaluation recommending that RTF is medically necessary at this time;

• Additional Psychiatric or Psychological evaluations if available.  This is to assist the Admissions Staff in developing a clinical and behavioral outline for presentation to the Glade Run Referral Committee;

• Treatment History; and

• Current grade and School District, SED label if applicable.
